Intake Paperwork will be send out for completion once an Initial appointment has been schedule, please complete it prior to your initial appointment. If you did not receive an e-mail please notified us. If Intake Paperwork are not completed at least 48 hours prior to your Initial appointment, your Intake appointment will be cancelled .
Please give us a call at (860) 799-1626 or e-mail at least 24 hours prior to your visit if you need to cancel or reschedule an appointment. If you do not show up for your scheduled appointment, and you have not notified us at least 24 hours in advance, you will be assessed a missed appointment fee. Excessive cancellations and no shows will result in a termination of services.
Do you take Insurance? Am I covered?
To determine if you have mental health coverage through your insurance carrier, the first thing you should do is call them. Check your coverage carefully. Some helpful questions you can ask them:
What are my mental health benefits?
What is the coverage amount per therapy session?
How many therapy sessions does my plan cover?
How much does my insurance pay for an out-of-network provider?
Is approval required from my primary care provider?
Do I have a Deductible? if so how much is my deductible?
